As a self-proclaimed injectable pro, it's safe to say I know my way around the benefits of natural-looking appearance medicine. As a patient of Joanna Romanowska for decades, I know what to expect with her gentle bedside manner. Calm, ultra-gentle injectable style and breadth of knowledge in the latest age-erasing treatments. This one starts with numbing cream under my eyes and upper lip for about 30 minutes. Initially, I thought I could tough this out without the cream, but the injections had a bite, and I was grateful for the numbing. Injecting small amounts of the salmon sperm closely together, all under the eye area, into the crow's feet, and then along the top of the lip area, once underway, the treatment was fairly speedy. I was sporting needle marks when I left, and my skin felt tight, but this side effect subsided by the early evening. My under-eye area was slightly red for the next few days, but easily covered with mineral makeup. This stimulating treatment is a slow burn; there is no instant change, so you need to measure your expectations with results. For someone who is very impatient, that is tricky, but this treatment takes time to stimulate collagen production and show any firming changes. Two treatments in, I will report back in a few months with before-and-after photos.

What are polynucleotide injections?
Polynucleotides are segments of DNA that have stimulatory and anti-inflammatory properties in tissues. When injected into the body, they stimulate repair and regeneration of the treated area. They are used for tendon repair, to accelerate surgical scar recovery, and to improve the texture of ageing skin.
Why is salmon sperm in the spotlight right now?
It is in the spotlight and for good reason. The injectable polynucleotides have been derived from salmon sperm DNA, which has been scientifically shown to have anti-ageing and regenerative properties, and are often referred to as a natural’ alternative’ to dermal fillers.
How do they work in the skin?
The mechanism is not completely understood, but Polynucleotides contain specific nucleotide sequences that match human cellular receptors, allowing them to integrate into repair mechanisms. Once injected, these molecules attract water molecules, creating a microenvironment that supports cellular activity while releasing growth factors that appear to promote healing and repair. However, the effects take two to three months for the results to become evident.
What skin types and ages will benefit from polynucleotide injections?
Because there is no shape change from the injections (unlike with hyaluronic fillers), they can safely be used on all skin types. For either delaying the ageing process in the younger patient, where there will be a very modest, if any, visible change, or stimulating a repair process with more obvious effects in the older or sun-damaged patient’s skin.
How do they differ from traditional fillers?
Because the results are from a bio-stimulatory process in the skin rather than a volumising effect in volume-depleted subcutaneous (beneath or under the layers of the skin) areas, they can be used in areas where shape change is undesirable. Unlike with most traditional fillers, the results take a couple of months after the last injection session to become fully evident. There is no instant gratification with this treatment.
What are the benefits, short and long term?
Initially, there may be some swelling following the procedure, which may smooth the skin. Still, the full effect of new collagen production, with tightening of the skin and possible improvement in dark circles under the eyes, takes longer to become evident. The results should last six to nine months, with individual variability.
How many treatments do you need, and how far apart are they?
We recommend repeating treatments monthly, with two to three treatments usually required, depending on the individual’s skin.
